Comments:
An unannounced renewal inspection was completed on July 31, 2019 from approximately 10:00a.m. to 12:30p.m. Forty-six children were in care and eight staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, outside play, snack time, and lunch time. Interviews and interactions were conducted and no children are currently taking medications. Five children?s records, six staff records and one corporate member record was reviewed. The Director/Owner was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of children?s records, the licensee failed to obtain a physical for one of five children before the child?s attendance or within one month after attendance. Evidence: 1. Child #5's record, enrolled on March 18, 2019, lacked a physical. 2. Administrator #1 acknowledged documentation of a physical has not been obtained.

Plan of Correction: The Director/Owner will obtain from the parent.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for two of five children. Evidence: 1. Child #2 and Child #4's records lacked complete emergency contact addressees. 2. Administrator #1 acknowledged this information was not documented.

Plan of Correction: The Director/Owner will obtain complete addresses from the parents.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records and interview, the licensee failed to maintain all the elements of staff?s records for one of seven staff. Evidence: 1. Staff #1's record, hired on June 6, 2019 as a lead teacher lacked documentation of the education required for the job position. 2. In an interview with Administrator #1, she reported that Staff #1 has an appropriate college degree but acknowledged that documentation has not been obtained.

Plan of Correction: The Director/Owner will obtain education records from the staff.

Standard #: 22VAC40-185-330-F
Description: Based on observation of the playground, the licensee failed to provide shade during the months of June, July, and August. Evidence: 1. Four classrooms were observed on the playground. One metal folding tent structure was observed without the canvas tent attached. Therefore no shade was provided, as this was the only means observed on the playground. 2. In an interview with Administrator #1 she acknowledged the center has the canvas tent available but that the teachers don't always use it.

Plan of Correction: The Director/Owner will require staff to use the shade tent daily.

Standard #: 22VAC40-185-340-D
Description: Based on observation, review of staff records and interview, the licensee failed to ensure that in each grouping of children at least one staff member shall be regularly present who meets the qualifications of a program leader or program director. Evidence: 1. A classroom of children, ages sixteen months to two yeas of age, was observed under the supervision of Staff #2 and Staff #3, while outside and in the classroom. A review of Staff #2 and Staff #3's records indicate that neither of these staff posses the education and/or experience required for a program lead. 2. In an interview with Administrator #1 shes acknowledged these two staff are assistants and reported that the regular program lead is on vacation all week.

Plan of Correction: This was corrected on site by switching staff.

Standard #: 22VAC40-185-350-C
Description: Based on observation and interviews, the licensee failed to ensure that when children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group. Evidence: 1. Staff #1 was observed on the playground, supervising fifteen children, ages four through thirteen years old, when the applicable ratio was 1:10. 2. In an interview with Administrator #1 it was determined that the four year child is her grandchild. Administrator #1 reported that this child "never" wants to stay in her designated classroom and that she regularly goes in the classroom with her older siblings.

Plan of Correction: This was correced on site.

Standard #: 22VAC40-185-550-M
Description: Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: 1. Twenty-two injury reports were reviewed. Eleven lacked the time the parent was notified of the injury. 2. Administrator #1 acknowledged these reports lacked this information.

Plan of Correction: The Director/owner will retrain all staff on the requirements of the reports.
