Comments:
An unannounced Monitoring Inspection was conducted today from 11:15am to 2:17pm. There were 79 (infants to School-Age) directly supervised by 21 staff. The physical plant, 8 staff records, 5 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on review, the facility did not have documentation of staff receiving the required orientation training their first day of assuming job responsibilities. Evidence: Reviewed 8 staff records and found that Staff 4 did not have documentation of orientation training.

Standard #: 22VAC40-185-260-A
Description: Based on review, the facility did not have documentation of an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: Reviewed the facility's records and found that they did not have documentation of an annual fire inspection report.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed peeling paint in the following classrooms: 1) Infants A 2) Pre-K 3) Twos

Plan of Correction: Maintenance is coming next Tuesday to correct, and another work order has been submitted.

Standard #: 22VAC40-185-440-E
Description: Based on observation, the facility failed to ensure that there shall be at least 12 inches of space between occupied cots, beds, and rest mats. Evidence: Observed in the Pre-K classroom, less than 12 inches between two sets of cots where children were sleeping. In one case the cots were touching.
