Comments:
The Licensing Inspector conducted an investigation in response to a self report incident as a result of a child found unsupervised in the hall bathroom on July 19, 2019. Interviews were conducted with staff and staff and child record reviewed. It was determined that the child was walking with the Preschool Summer camp class up the stairway and left the group. The teacher reported it to the office immediately. The child was not within sight and sound supervision for approximately 1-2 minutes. The information gathered during this investigation supports the allegations; violations were discussed with the center director. Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not obtain tuberculosis (TB) screening results at least every two years from the date of the first initial screening. Evidence: 1. Staff A (Date of hire 7/21/2014) had a TB test result dated 4/24/2017.

Plan of Correction: During the staff file review anyone with expired TB tests will be required to immediately set-up an appointment and bring in documentation that the test was started, as well as documentation that the test was completed. Staff will have 5 business days to start the process.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were not complete. Evidence: 1. Staff A (Date of hire 7/21/2014) did not have written documentation to demonstrate that the individual completed orientation training and staff development.

Plan of Correction: Staff files will be reviewed and any with missing documentation will have that documentation redone and dated (with a note that the original is missing).

Standard #: 22VAC40-185-90--A
Description: Based on record review, a written agreement between the parent and the center was not in the child's record by the first day of the child's attendance. Evidence: 1. Child #1 (start date 6/03/2019) did not have a signed written agreement regarding authorization for emergency medical care, picking up an ill child as soon as possible if so requested by the center and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.

Plan of Correction: Pre-School Camp will add an additional parent signature page to cover all agreements that were missing (emergency medical treatment, parent handbook acknowledgement, notification of communicable diseases, and addresses for emergency contacts.

Standard #: 22VAC40-185-240-C
Description: Based on record review, all staff working directly with children did not complete 16 hours of training. Evidence: 1. Staff A (Date of hire 7/21/2014) did not complete 16 hours of training annually for the time period 7/21/2018 to 7/21/2019.

Plan of Correction: Teachers will turn in their training hours on the anniversary of their start date, or when they leave the CDC (whichever comes first).

Standard #: 22VAC40-185-340-F
Description: Based on multiple staff interviews, children under 10 years of age were not always within actual sight and sound supervision of staff. Evidence: 1. On July 19, 2019 at approximately 11:30am, Child #1 (3 year old) was reported missing for approximately 1-2 minutes from the Preschool Summer Camp. A staff member found the child in the hallway bathroom and brought the child to the front office. 2. Staff A stated that she did a head count of the children she was supervising (three year olds) before entering the stairs and had counted 10 children. When she exited the stairs on the 2nd floor, she did another head count and had 9 children. She immediately notified the Staff C with a walkie talkie. Staff A estimated the child was missing for a very short time/split second. 3. Staff B stated he entered the hall bathroom and found child #1 in the bathroom. Staff B stated he took child #1 to the office. As Staff B walked into the office with the child, Staff C had already been notified the child was missing.

Plan of Correction: Any teacher transitioning on their own with more than 4 children will use the elevator when moving between floors. Any teacher transitioning on their own with 5, 8, or 10 children (whichever is the maximum for their age ratio), will radio the office for an assistant if one is not readily available within their vicinity.
