Comments:
The licensing inspector conducted an unannounced focused monitoring on Friday, August 9, 2019 between 11:00 am to 1:50pm determine the provider's compliance with licensing standards at the child day center. During this inspection, there were 51 children and 11 staff present at the center including administrative staff. The licensing inspector completed observations of the children in care participating in a variety of activities to include transitioning indoors from outdoor activities, engaging in free play, tummy time, participating in indoor activities, playing cards, interacting with staff and peers, and eating lunch. The children's lunch today consisted of hot dogs in croissants, broccoli, mixed fruit, and milk/water. The center's playground areas and designated children care equipment and areas were inspected. There were rechecks of three children's and four staff's records reviewed during this inspection. Standard #: 22VAC40-185-70-A
Description: Based review of staff records, the center did not ensure to obtain all staff required information. Evidence: In Staff#1's record (date of hire: 7/29/19), the date of contact and signature of person making call was not documented for both references.

Plan of Correction: The center will ensure staff conducting references checks will be sure to sign and date upon completion.

Standard #: 22VAC40-185-280-B
Description: Based on observations, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the Caterpillar's 1 classroom, the supply closet was unlocked containing three containers of paint primer and multiple shaving creams with the warning labels "keep out of reach of children". 2. There was a container of pain medication with the warning label located on the upper shelf in the classroom. 3. Exhibit A and Exhibit B photographs taken of items found inside the closet and on the classroom shelf during the inspection on August 9, 2019.

Plan of Correction: Administration will retrain staff on the importance of keeping all hazardous substances locked.
