Comments:
An unannounced monitoring visit and facility tour was conducted from 2:15pm-4:00pm. At the time of inspectors' arrival there were 10 preschool to school aged children in care with 2 staff members and 1 administrator. Children were observed participating in instructional time, and transitioning between activities. A sample of 5 children's records and 4 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are detailed on the violation notice and were discussed with the Director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review and interview it was determined staff did not resubmit TB test results at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department. Evidence: 1. The record for staff #1(hired 7/11/16) did not contain current TB test results. The documentation available was dated 7/21/17.Staff #1 confirmed they had not gotten a test or screening since 7/21/17. 2. The record for staff #4(hired 7/11/16) did not contain current TB test results. The documentation available was dated 7/19/17.Staff #1 confirmed they had not gotten a test or screening since 7/19/17.

Plan of Correction: Staff will be screened or retested for TB.

Standard #: 22VAC40-185-510-N
Description: Based on medication review it was determined the center did not dispose of expired medications that parents do not pick up within 14 days. Evidence: In the Center's medication cabinet there was: 1. Medication for child #6 expired 6/2019 2. Medication for child #7 expired 3/2019

Plan of Correction: The Director will return medications to parents.

Standard #: 22VAC40-185-550-M
Description: Based on record review and interview it was determined the center did not ensure that each written record of children's serious and minor injuries contained all of the required components. Evidence: 1. The injury reports for child #8 dated 6/28/19 and 7/1/19 did not contain the date and time of parental notification. 2. The injury report for child #9 dated 6/17/19 did not contain the date and time of parental notification.

Plan of Correction: Director will retrain staff on completing injury reports correctly.
