Comments:
An unannounced monitoring inspection was conducted today between 10:00 am and 2:30pm. There were 53 children in attendance directly supervised by 11 staff members. The physical plant, staff records,, children's records, emergency plans, emergency supplies, medications, injury reports and evacuation drills were inspected. Children were observed having breakfast, singing and coloring. There was an abundant supply of materials for the children. There was an adequate number of teachers with current first aid/cpr, MAT certification and DOH certification present. Areas of non compliance are cited on the Violation Notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of 5 children's records, the center failed to ensure that all children have a physical before attendance or by the 30th day of attendance. Evidence: Child #2's record does not show documentation of a physical by the 30th day of attendance.

Plan of Correction: I will contact parent's and get the physical and place a copy in child's record.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the provider did not maintain complete children's records. Evidence: Child #4's record does not contain documentation of name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: I will contact parent's today and place documentation in the file.

Standard #: 22VAC40-185-350-E-3
Description: Based on observation, the center failed to ensure a 1:8 ratio ratio in the 2 year old room. Evidence: During the inspection and observation, the two year old room had 1 teacher for 9 children on 8/13/2019

Plan of Correction: I will remind the teacher of maintaining ratio.

Standard #: 22VAC40-185-440-B
Description: Based on observation, the center failed to ensure that cots will be identified for use by a specific child. Evidence: The cots in the 2 1/2 year old room and in the 3's classroom were not labeled for individual use.

Plan of Correction: We will have teachers label

Standard #: 22VAC40-185-550-M
Description: Based on a review of 20 injury reports, the center failed to ensure that the injury report had parent and staff signatures or the signatures of two staff members. Evidence: 16 out of 20 injury reports reviewed had no signature or one signature.

Plan of Correction: We will do a new form and remind staff to have parents sign injury reports and if parents don't sign injury reports, the report should be signed by two staff persons.

Description: Based on observation, the center failed to ensure that the most recent inspection notice is posted. Evidence: The center did not have a Licensing Inspection Notice posted.
