Comments:

An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:00 AM and departed at 1:00 PM. Forty-one children were in care. Morning activities, lunch service and nap time were observed. Medication administration and injury records were reviewed. A sample size of six staff records and five children's records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Standard #:	22VAC40-185-130-B
Description:	Based upon review of five children's records and staff interview, the facility has not ensured that they have obtained documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The most recent immunization documentation for child 2, aged 7 months, is dated 12/28/2018. 2. Staff 1 verified that an additional immunization record is needed for child 2.
Plan of Correction:	The facility responded with the following: Immunization records for children under age 2 years will be tracked to ensure that updates are obtained every six months.
Standard #:	22VAC40-185-140-A
Description:	Based upon review of five children's records and staff interview, the facility has not ensured that each child has a physical examination before attending or within one month after attending. Evidence: 1. The record for child 3 indicates that child 3 began attending on 2/12/2018. The record does not include documentation of a physical examination. 2. The record for child 4 indicates that child 4 began attending on 4/5/2019. The record does not include documentation of a physical examination. 3. Staff 1 verified that the records for child 3 and 4 do not include documentation of physical examinations.
Plan of Correction:	The facility responded with the following: All children's records will be reviewed to ensure that documentation of physical examinations are received within the required time frames.
Standard #:	22VAC40-185-70-A
Description:	Based upon review of six staff records and staff interview, the facility has not ensured that each staff record includes the name, address and telephone number of an emergency contact person. Evidence: 1. The record for staff 3 did not include the name and address of an emergency contact person. 2. The record for staff 6 did not include the address of the emergency contact. 3. Staff 1 verified that the emergency contact information was not in the records of staff 3 and staff 6.
Plan of Correction:	The facility responded with the following: Staff 6 completed the information during the inspection. Staff 3 will complete the information when she arrives for duty today. The emergency contact form will be adapted to more clearly request the needed information.
Standard #:	22VAC40-185-280-B
Description:	Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children. Evidence: There is a container of hand soap on the children's hand washing sink in the preschool classroom. The container is labeled "warning, keep out of reach of children".
Plan of Correction:	The facility responded with the following: A wall mounted single pump soap dispenser will be purchased and placed on the wall by the sink.
