Comments:
An investigation of a self-reported incident was conducted on 7/2/2019. The inspector arrived at 7:30 AM and departed at 10:45 AM. The facility self-reported that a three year old child was left on her cot in the classroom when the children were evacuated for an unannounced evacuation drill. The investigation included review of the child's record, review of staff records, review of documentation of the incident and staff interviews. A violation was cited under sight and sound supervision.

Description: Based upon staff interview and review of documentation, the facility failed to ensure that children under ten years of age are always within the actual sight and sound supervision of staff. Evidence: 1. The facility self-reported that on 6/26/2019, during an unannounced evacuation drill, child 1, three years old, was left on a cot in the upstairs classroom when the remaining 14 children were evacuated to the playground by the two staff on duty with the class. 2. Documentation of the incident recorded that upon reaching the playground, staff 1 reported to staff 2, the assistant director, that child 1 was not on the playground. Staff 2 returned to the classroom to find child 1 on her cot in the classroom. 3. In interview, both staff 2 and staff 3 confirmed the above scenario. 4. Staff 2 reported that the entire evacuation drill (including bringing child 1 outside to the playground area) took a total of 3 minutes and 5 seconds.

Plan of Correction: The facility responded with the following: We self-reported this incident. In response to the sited violation, we followed all corrections suggested by our state licensing agency and out franchise consultants. We updated our evacuation plan to include specific staff supervision positions and tasks during evacuation drills. During evacuations, a teacher will be identified as the room sweeper to do a check of the room before being the last person to exit the room. A member of Administration will also be on each floor of the building to conduct a final sweep of all classrooms as the children and staff are exiting. To ensure understanding of evacuation drills and supervision policies, a mandatory staff meeting was held to retrain all staff members. A fire drill was also completed successfully, without incident. Practice evacuaiton drills have been scheduled for extra training.
