63.2(17) License & Registration Procedures

Comments:
An unannounced Monitoring Inspection was conducted on August 5, 2019 from approximately 5:15 pm until 5:40 pm. Upon arrival there were 36 children in care 4 with staff. Additional staff were present for administrative purposes. Infants were observed eating snack, freely playing and during the diapering process. Children were observed playing in centers to include home living, dolls and reading.
The sample size consisted of 6 children's records and 4 staff records. The results of the inspection were reviewed and verified with the center owner during the exit interview. Areas of non-compliance are noted on the violation notice. Please contact the licensing inspector, Christine Mahan (757) 404-0568 with any questions.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review of 4 staff records the licensee did not ensure each staff had at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow up tuberculosis screening.

Evidence: The record for staff #1 did not include an updated TB screening as the most recent screening is dated 7-28-17.

Plan of Correction: Staff members did have T.B. administered on 8/12/19 and will be read and returned 8/14/19.

Standard #: 22VAC40-185-60-A
Description: Based on record review of 6 children's records the licensee did not ensure each child has a record with all required information.

Evidence: Two of the children's records had the following missing information as confirmed by the center owner.
A) The record for child #3 did not include the street name, house number, the state and zip code of both emergency contacts listed.
B) The record for child #5 did not include the street name or house number for one emergency contact listed.

Plan of Correction: Director spoke to the parents of each child and the files were corrected the same day.

Standard #: 22VAC40-185-260-A
Description: Based on documentation review and staff interviews, the licensee did not ensure that the center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

Evidence: There was not annul fire inspection conducted, as the most recent inspection available for review was dated 2-15-2018.

Plan of Correction: The center was inspected on 2/22/19 and for some reason the Fire Marshall did not submit a report. When the director contacted the new Fire Marshall on 8/6/19 he scheduled a visit the next day, inspected 8/7/19 -no violations.

Standard #: 22VAC40-185-500-A
Description: Based on observation and staff interviews the licensee did not ensure that hand washing occurred as required for children and staff.

Evidence: The following observation were made and center staff and children did not wash their hands as required.
A) Observation of two children (child #2 and child #3) in the infant room was made during their snack time routine and their hands were not washed with soap and running water or disposable wipes before and after eating meals or snacks.
B) Observation of one child in the infant room (child #3) and one child in the two's room (child #1) was observed during the diaper changing process and the child's hands were not washed with soap and running water after diapering.
C) Observation of one child in the infant room (child #3) and center staff was made during the diapering process and the center staff (staff #4) did not wash their hands with soap and running water before and after helping a child use the toilet or a diaper change.
D) Observation of staff #4, in the infant room was made when they helped two children child #2 and child #3 with feeding and they did not wash their hands with soap and running water before and after helping with feeding.

Plan of Correction: Director spoke individually and as a group to remind staff and review regulations. Staff very nervous when being observed by the state. I reminded them even though they are wearing gloves at diapering, they must wash their hands afterwards. I posted diapering and hand washing procedures in the infant-toddler room and the 2 year room as a reminder when get nervous.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interviews, the licensee did not ensure the diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.

Evidence: Observation were made of the diapering process in the infant room and two's room.
A) After diapering child #3 in the infant room, the center staff did not use soap and water to clean clean or a sanitizing solution to sanitize the diaper pad.
B) After diapering child #4 in the two's room, the diaper pad was sprayed with a bleach and water solution and was immediately wiped down and was not left to air dry. The diaper pad was not cleaned and was not sanitized.

Plan of Correction: Director reviewed diapering procedures with staff. Posted step by step instructions if they get nervous when a licensing specialist is here, they can refer to post.

Standard #: 22VAC40-185-560-K
Description: Based on observation and staff interviews, the licensee did not ensure tables and high chair trays shall be sanitized before and after each use for feeding.

Evidence: Two infants were observed eating in highchairs and both highchairs were not sanitized after eating as they were only wiped down with a towel and water. Sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution. The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution.

Plan of Correction: Director reminded staff that even though the highchairs were cleaned. They must, remember to "sanitize" . Director then went over and shared the state definition.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review of 4 staff records, the licensee did not ensure that all staff had obtained a finger print check prior to their first day of employment.

Evidence: The record for staff #4 included a finger print check dated 7-12-2019 and the first date of date employment/ orientation was dated 7-8-2019.

Plan of Correction: I did do paperwork for the file before the staff member reported to work with children. In the future all paperwork with children. In the future all paperwork will be done after finger print investigation.
