Comments:
A Subsidy Health and Safety Inspection (SHSI) was conducted on August 8, 2019 to review supplemental health and safety requirements. The inspection was from approximately 2:15 pm until 5:40 pm. Upon arrival there were 36 children in care 4 with staff. Additional staff were present for administrative purposes. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. The results of the inspection were reviewed and verified with the center owner during the exit interview. Standard #: 22VAC40-665-540-A
Description: Based on record review of 4 staff records, the vendor did not ensure each staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department

Evidence: The record for staff #4 included a TB screening dated 6-7-19 and date of employment was 7-15-2019 and was conducted more than 30 days prior to employment.

Plan of Correction: Driver will ensure in the future that new employers will not receive TB's earlier s that 30 days of employment.

Standard #: 22VAC40-665-650-E
Description: Based on record review, the vendor did not ensure a written policy was developed and implemented that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.

Evidence: There was not a written policy and procedures that described how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. This missing written policy was confirmed by the center owner.

Plan of Correction: Director corrected this the next day and wrote a policy with this information listed for the future.
