Technical Assistance:
Technical assistance discussed today included record keeping. Make sure to only use the forms that are meant for Centers, not family day homes.

Comments:
An unannounced monitoring inspection took place today from 10:30 am - 1 pm. There were 12 children present with two staff. I observed children listening to a video, painting pretend "ice cream cones" with pastel paint, singing about the days of the weeke and the alphabet; eating lunch and preparing for rest time. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records today, one child did not have a physical exam completed. Evidence: Child # 3 had a copy of immunizations but no proof of a physical exam as required.

Plan of Correction: We will talk to the parent.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's files, the following information was not complete: 1. Child #1's parents did not signed the agreements as listed on the Child Day Center form because an Family Day Home form was used instead, and the information was over a year old, dated 3/18. 2. Child # 2 had an allergy documented by the parents but no information on action to take in the event of an allergic emergency.

Plan of Correction: The parents will always fill out the form for the Child Day Center and we will always make sure they fill out the "action to take in the event of an allergic emergency" or write if it is a preference due to a family history.

Standard #: 22VAC40-185-240-D-1
Description: Based on observation and staff confirmation today, there were no staff present today who have been trained in Medication Administration Training. Evidence: Child # 3 had two allergy medications present but no staff on the premises this morning were MAT trained.

Plan of Correction: The teachers are going to MAT training in September.

Standard #: 22VAC40-185-510-A
Description: Based on a review of records today, there was no authorization to administer medication as required for medication brought in by parents. Evidence: Child # 3 had an Epi-pen present and a bottle of anti-allergy medication for a documented allergy, but there were no authorizations documented by the parents.

Plan of Correction: We will get an authorization from the parent today. We gave the paper but it was not returned.
