Technical Assistance:
The hot water in one of the infant rooms felt too hot to the touch, but my thermometer was not operational today. Please check it to make sure it is under 120 degrees. The evacuation crib is dated 12/11, but it is recommended that you obtain documentation from the manufacturer to make sure it meets the current criteria for crib safety. (rules were change 12/11).

Comments:
An unannounced monitoring inspection was conducted today from 10:30 - 1:45 pm. There were 40 children present. All rooms were within ratio. I observed children playing on the blacktop area and playground before coming in for lunch. Record keeping was well-organized. The playground was not inspected due to mid day storms. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, there were some areas of the center that were not maintained in a clean, safe and/or operable condition. Evidence: In baby areas, there were dangling cords near the infant chairs that were accessible to children. A low installed window air conditioning unit had an accessible cord and hose, and some sharp areas near the top. There were some splintery areas accessible and chipped paint accessible: on the front door and in areas of the baby rooms. In the hallway, several hung paintings and framed works were loose and need to be made secure. (A lower tab is easily moved by those walking by.)

Plan of Correction: We will report to the church the chipping paint and talk about ways to make the frames more secure. We can cover the sharp areas with masking tape and tape the cord with duct tape.

Standard #: 22VAC40-185-510-N
Description: Based on observation during today's inspection, one medication was not returned to the parent after it was no longer in use. Evidence: There was a tube of ointment for Child # 2, but staff report it is no longer being used. No documentation was available.

Plan of Correction: We will return it to the parent today.

Standard #: 22VAC40-185-550-C
Description: Based on observation during today's inspection, the emergency procedure maps did not include all required items. Evidence: The posted emergency maps did not show an alternative exit route if the main exit is blocked, and there were no indications of the location of the shelter-in-place area.

Plan of Correction: We will update those.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on a review of staff records today, not all had the most recent Sworn Statement form. Evidence: All of five files reviewed today had older Sworn Statement forms that do not currently meet the standards.

Plan of Correction: We will print out and get staff to sign new Sworn Statements.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of staff records today, there was information on one staff who indicated they lived in another state in the past five years, but there was no record of a Search of the Central Registry for that state. Evidence: Staff #3 lived in MD in the past five years, but there was no search conducted for that state as required.

Plan of Correction: She will fill out the forms as soon as possible.
