Comments:
An unannounced renewal inspection was conducted from 10:00 am - 2:00 pm with the director. There were 67 children in care, ranging from five years to 16 years of age, supervised by 7 staff. The children were observed playing basketball in the gym, reading books, playing video games and eating lunch which is brought from home. Five children's records were reviewed and in compliance. Five staff records were reviewed, four buses were reviewed and three medications and authorization forms were reviewed. The emergency escape drill log, first aid kit, battery operated radio and flashlight were all observed and in compliance. The last health inspection was: 7/13/2018 and the last fire inspection was: 6/3/2019. Standard #: 22VAC40-185-70-A
Description: Based on a review of five staff files, and an interview with the director, it was determined that the facility did not have all required information in each staff file. Evidence: For staff #2 (date of hire: 11/23/15 there was no documentation of two references as to character and reputation as well as competency before employment, or written information to demonstrate the individual possesses the education, orientation training, certification, or experience required by the position.

Plan of Correction: Will provide orientation training to staff tomorrow. Will obtain references immediately.

Standard #: 22VAC40-185-80-A
Description: Based on a review of the written attendance record and staff interview(s), it was determined that the facility did not ensure all children were included on the written attendance record. Evidence: The center did not have a written record of attendance for any of the ten groups of children, documenting who was present.

Plan of Correction: Will create and implement an attendance sheet for each group of children.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the facility failed to have children under the age of 10 within actual sight and sound supervision of staff. Evidence: an eight year old child was observed walking down the steps by himself. Staff on the first floor escorted him back upstairs to his classroom.

Plan of Correction: Will retrain staff on sight and sound supervision.

Standard #: 22VAC40-185-510-C
Description: Based on a review of three medications, it was determined that the center did not follow required procedures. Evidence: there is no medication authorization form with parent and physician authorization for two medications for Child #1. The medication authorization form for child #3's medication authorizes Prince William County School personnel to give the medication. There is no written authorization for Adventureland staff to administer the medication.

Plan of Correction: Will obtain the missing medication authorization from parent and physician.
