Comments:
An unannounced monitoring inspection was conducted today for 9:45am until 2:30pm. There were 61 children with 14 staff directly supervising children. The children were observed participating in a gym program, playing with table toys, sitting in carriers and outside water play. The physical plant, children's records, staff records, menu's, evacuation plans, emergency supplies and medications were inspected. Areas of non-compliance are cited on the Violation Notice. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of 5 staff records, 3 of 5 staff records were incomplete. Evidence: 1) Staff #2, #3 and #5 have no documentation that Child Abuse and Neglect Training has been completed. 2) Staff #5 is missing documentation of a second reference check.

Plan of Correction: Training will be completed today and documentation will be placed in staff records. I will get reference information and documentation will be placed in file.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure that areas and equipment of the center, inside and outside shall be maintined in a clean, safe and operable condition. Evidence: 1) In the infant room the paint is chipped off the wall near the infant book bin. 2) Three of the blue floor mats used in the Infant Room have frayed corners and edges. 3) In the Infant Room the orange curved floor toy is frayed at the edges and has a rip in the material. 4) In the Toddler Room, there is paint chipped off the wall near the entry door frame. 5) in the Toddler Room, there is a gouge in the wall near the back door and the paint is chipping off. 6) In the Jr. Preschool Room, chipped paint on side wall near electrical outlet. 7) On the playground, there was a nut missing to a bolt on the 1st large blue play structure.

Plan of Correction: Frayed mats will be discarded. Torn play structure in Infant Room will be discarded. Chipped paint will be covered with contact paper until we can paint.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that a nonabsorbent surface for diapering or changing will be used. Evidence: There is a tear in the pastel blue changing pad used in the Infant Classroom.

Plan of Correction: Discarded during in inspection.

Standard #: 22VAC40-185-510-C
Description: Based on observation and inspection of the center, the provider did not ensure that the center's procedures for administrationg medication shall have methods to prevent use of outdated medication. Evidence: Child #6 's medication expired 7/2019.

Plan of Correction: Parents will be dropping off up to date medication.

Standard #: 22VAC40-185-510-N
Description: Based on review of medications in center, the center failed to ensure that when an authorization for medication expires, the parents shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Evidence: Child #6's medication authorization expired 08/10/2019.

Plan of Correction: Contacted the parent's they will pick up medication today and return with new paperwork.
