Comments:
An unannounced monitoring inspection was completed on 08/14/2019. There were 19 children, ages 4 years-10 years, and 2 staff members present during the inspection. The inspector reviewed 5 children's records, the on-site staff record requirements for 2 staff members and one medication during the inspection. The complete staff records will be reviewed on a later date at the corporate office. Children were observed in the following activities: arriving to the program via public school transportation, having an afternoon snack, during homework time, in free choice activities and going outside to play. Proper hand washing and table sanitization were also observed. The inspector reviewed the emergency supplies during the inspection. The inspector discussed the following with the staff: subsidy requirements and children's allergies. The inspector arrived for the inspection at 3:00 p.m. and departed at 5:15 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements required by the standards. Evidence: The record for Child #1 did not contain the following information: name and phone number of the child's physician and the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the address for the second designated person was incomplete).

Plan of Correction: The staff will get the information from the parent.

Standard #: 22VAC40-185-70-A
Description: Based on discussion with the staff, the facility failed to ensure that staff records contained all of the elements as required by the standards. Evidence: There was no documentation of a name, address and telephone number of a person to be notified in an emergency which shall be kept at the center. Staff #1 and Staff #2 verified that the required information was not available during the 08/14/2019 inspection.

Plan of Correction: Staff #2 will get with the Child Care Coordinator.

Standard #: 22VAC40-185-340-C
Description: Based on observations and discussion with the staff, the facility failed to ensure that there always shall be on the premises and on field trips when one or more children are present, one staff member who meet the qualification of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. Evidence: Staff #1 was the only staff member present when the 19 children arrived at 3:16 p.m. via the public school bus. Staff #2 arrived to the program at 3:28 p.m.

Plan of Correction: Always have 2 staff present.

Standard #: 22VAC40-185-350-C
Description: Based on observation, attendance documents and discussion with the staff, the facility failed to ensure that when children are regularly in ongoing mixed age groups, the staff-to-child ratio applicable to the youngest child in the group shall apply to the entire group. Evidence: 1. Staff #1 was the only staff member present with 19 children, ages 4 years-10 years, arrived to the center via public school bus. 2. Child #2 was identified at the youngest child in care; the review of Child #2's record determined that Child #2 was 4 years old and would not be turning 5 years before September 30. 3. Staff #1 stated that the children are always only in one group each day. 4. The staff-to-child ratio would be based on the youngest child in the group, which was determined to be 4 years old. The staff-to-child ratio for children from three years to the age of eligibility to attend public school, five years by September 30 is one staff member for every 10 children.

Plan of Correction: Have to staff members present. Always be in ratio.

Standard #: 22VAC40-185-550-C
Description: Based on observations and discussion, the facility failed to ensure that emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: There were no emergency evacuation and shelter-in-place procedures posted at the facility. The map was posted but there were no procedures posted.

Plan of Correction: The staff will discuss with the Child Care Coordinator.
