Comments:
An unannounced monitoring inspection was conducted between the hours of 10:00am -1:00pm. Upon arrival there were 111 children in care with 18 staff present. Fourteen children were on a field trip with 3 staff. A sample of 9 children and 2 staff records were reviewed. Children were observed during lunch, nap and .....

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review, the center did not ensure all required information was maintained in each enrolled child's record. Evidence: Four month-old child #1 (enrolled doe 06/09/19) did not have the name, address or phone number for the child's father. Staff #1 confirmed The child's information was the father was not in the record.

Plan of Correction: The following corrective action was provided by the designated person-charge during the exit interview: The director will follow-up with the child's parent and obtain all required information.

Standard #: 22VAC40-185-70-A
Description: Based on a record review the center did not ensure all required information was kept for each staff person. Evidence: 1. Staff #1 (hired on 10/05/17) was listed as a Teacher Assistant in the record. Staff #1 confirmed, her title is not a Teach Assistant, and was promoted to Assistant Director on 06/18/18.

Plan of Correction: The following corrective action was provided by the designated person-charge during the exit interview: This was an oversight. The record was corrected during the inspection.

Standard #: 22VAC40-185-370
Description: Based on observation the center did not ensure all daily activities for infants were met. Evidence: In the infant room Staff #3 placed 6 month-old baby, child #9, in her assigned crib on her stomach.

Plan of Correction: The following corrective action was provided by the designated person-charge during the exit interview: The Assistant Director will talk to the director and district manager about safe sleep and will monitor.

Standard #: 22VAC40-185-560-F
Description: Based on observation the center did not ensure all requirements were met when providing food. Evidence: In the school-age room children were observed eating lunch. There was not a staff member observed sitting with the children.

Plan of Correction: The following corrective action was provided by the designated person-charge during the exit interview: Staff will be reminded that they should be sitting with the children during meals. Administration will monitor.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a record review the center did not ensure all staff that have listed living outside the state of VIrginia within the past 5 years, had an out-of-state Central Registry Check from that state. Evidence: Staff #1 (hired 10/05/17) listed having lived outside the state of Virginia within the past 5 years. Staff #1 did not have documentation of having the results of a central registry check from that state. Staff #1 confirmed the results of the out-of-state central registry check was not in the record.

Plan of Correction: The following corrective action was provided by the designated person-charge during the exit interview: It was sent out, however, it was never returned. The Director will follow-up and resend.
