An unannounced monitoring inspection was conducted from 10:45am-12:45pm. At the time of entrance, 44 children were in care with 9 staff members present. Children were observed during outside time and water play. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, evacuation drills and attendance records were reviewed. Standard #: 22VAC40-185-160-C Description: Based on review of six staff files, the center did not obtain the results of a follow up tuberculosis screening every 2years. Evidence: 1. Staff #3 had tuberculosis screening results dated 12/21/2016. Plan of Correction: The staff member will get an appointment to get this updated.
Standard #: 22VAC40-185-60-A Description: Based on record review, four of five children's records did not have documentation of child updates and confirmation of up-to-date information in the child's record. Evidence: 1. Child #1's last annual update confirming the child's information in the child's record was up to date was signed 7/24/2018. 2. Child #2's last annual update confirming the child's information in the child's record was up to date was signed 1/29/2018. 3. Child #3's last annual update confirming the child's information in the child's record was up to date was signed 8/07/2017. 4. Child #5's last annual update confirming the child's information in the child's record was up to date was signed 6/2018. Plan of Correction: I will have the parent's review and sign the updates today.
