Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 07/11/2019 regarding behavior guidance, background checks and staff qualifications. The inspection was initiated on 08/15/2019 and completed on 08/19/2019. On 08/15,2019 the inspector was on site from 1:28pm to 2:55pm and on 08/19/2019 from 9:45am to 11:30pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding out of state background checks. See the violation notice for more details. Description: Based on record review, the center did not ensure that two of three staff records contained documentation of out of state child abuse and neglect search request by the end of the 30th day of employment and a follow up on an out of state child abuse and neglect search 45 days after request to search. Evidence: 1. The record of staff #1 (start date: 8/3102018) had documentation of an out of state search request dated 11/20/2018, the center did not have documentation of a follow up on the request. The record of staff #1 also indicated that the staff lived in two other states and the center did not obtain documentation of the out of state child abuse and neglect search requests. 2. The record of staff #2 (start date: 2/18/2019) had documentation of two out of state search requests dated 2/19/19 and 8/17/2019, the center did not have documentation of a follow up on the requests. The record of staff #2 also indicated that the staff lived in one other state and the center did not obtain documentation of the out of state child abuse and neglect search request.

Plan of Correction: All original and follow up e-mails will be mailed out no later than the 23rd. I will scan all signed documents to the inspector.
