Technical Assistance:
Technical assistance was provided regarding qualifying lead teachers, what needs to be covered in the 24 hours of training, reference check requirements and shelter in place documentation.

Comments:
An unannounced renewal inspection was conducted today from 9:00am-12:30pm. There were 39 children supervised by 7 direct care staff. The physical plant, programming, 10 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, going outside, reading books, and free play. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-160-C
Description: Based on review of 10 staff records, 1 does not have results of a follow up tuberculosis screening. Evidence: The current results of a TB screening for staff#1 expired 5/24/19.

Plan of Correction: I will have her go this week.

Standard #: 22VAC40-185-70-A
Description: Based on review of 10 staff records, 1 in incomplete. Evidence: Staff #2, start date 7/18/19, does not have documentation of qualifications for the position of lead teacher. References are not complete for staff #2, missing information include who conducted the reference check and name of business contacted.

Plan of Correction: References were corrected during the inspection, and going forward we will make sure to fill form out completely. We will complete 24 hours of lead teacher training and place documentation in her record.

Standard #: 22VAC40-185-510-G
Description: Based on review of 3 medications, 1 has an incorrect medication on site. Evidence: Child #1 has an authorization for a name brand medication and a generic medication on site.

Plan of Correction: I will give the parent a new authorization for the doctor to complete. Going forward we will make sure to ensure that whatever is written on the form, is the medication that parent brings in.
