An unannounced monitoring inspection was conducted today 8/19/2019 beginning at 6:20 A.M. and ending at 8:20 am. There were 50 children present today with seven staff. The inspector reviewed five children's and seven staff's records during the inspection. The children were observed during: arrival, free play, and game time.

Standard #: 22VAC40-185-160-A Description: Based on review of seven staff's records, the center failed to ensure that all TB screenings or test were completed within 21 days of hire or completed within 12 months prior to starting employment. Evidence: 1. Staff # 2 started on 2/18/2019 and the TB test/screening was dated 4/13/2019. Plan of Correction: In the future all TB test/screenings will be completed prior to starting.
Standard #: 22VAC40-185-210-A Description: Based on review of seven staff's records, the center failed to ensure that all program leaders had the required documentation needed for the job position. Evidence: 1. Staff # 1 was missing a copy of the high school diploma or equivalent. Plan of Correction: The site director will have the staff person to obtain a copy of the high school diploma or equivalent.
Standard #: 63.2(17)-1720.1-A Description: Based on review of seven staff's records, the center failed to ensure that all staff had the finger print eligibility letters returned prior to starting their position. Evidence: 1. Staff # 3 started on 5/17/2019 and the finger print eligibility letter was dated 5/20/2019. 2. Staff # 4 started on 4/20/2019 and the finger print eligibility letter was dated 4/24/2019. 3. Staff # 6 started on 5/17/2019 and the finger print eligibility letter was dated 5/22/2019 Plan of Correction: In the future all staff will not start in the position until the finger print letter has been returned.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of seven staff's records, the center failed to ensure that all staff that have lived out side of the state of Virginia in the past five years had a out of state CPS background check requested. Evidence: 1. Staff # 3 and 5 both indicated that they have lived out side of Virginia in the past five years. Staff # 3 and # 5 started on 5/17/2019 and no request has been obtained. Plan of Correction: The site director will work with the child care director to obtain the out of state CPS checks within 10 days as stated.
