Comments:
An unannounced Monitoring Inspection was conducted today from 10:30am to 1:20pm. There were 72 (birth to school-age) directly supervised by 12 staff. The physical plant, 10 staff records, 10 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, preparing for lunch, and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 10 children's records and found that 1 (Child 7) did not have documentation of parent's annual review/update of children's emergency contact information.

Plan of Correction: Parent was given a re-registration form on 8/12/19.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed in the Toddler classroom a small hole in the wall, low enough and accessible, that children could fit fingers through.

Plan of Correction: Small hole is currently covered making it inaccessible to children. A maintenance request has been submitted for repair.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to children. Evidence: Observed in the Older Twos classroom, a key in a cabinet door, that was not locked, with sanitizer and peroxide multi-surface cleaner accessible to children.

Plan of Correction: Corrected at the time of the visit.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the facility failed to ensure that electrical outlets have protective covers. Evidence: Observed in the three year old classroom, 2 uncovered outlets in the boys bathroom and 2 uncovered outlets in the girls bathroom.

Plan of Correction: Corrected at the time of the visit.

Standard #: 22VAC40-185-430-B
Description: Based on observation, the facility failed to ensure that the materials and equipment available are age and stage appropriate. Evidence: Observed in the Toddler classroom, a large number of staples in the wall accessible to children in the classroom, and several of the staples were loose and able to be removed from the wall.

Plan of Correction: All staples have been removed from the wall. Staff have been retrained on regulation.

Standard #: 22VAC40-185-550-D
Description: Based on review, the facility failed to ensure that they conduct monthly evacuation drills and a minimum of two shelter-in-place practice drills per year. Evidence: Reviewed the emergency evacuation drills and found that there was only 1 shelter-in-place drill conducted in the last 12 months (3/19/19).

Plan of Correction: A shelter in place drill was held on 8/13.
