An announced non-mandated record review inspection for the YMCA Ferrum Elementary After School Program was completed on 08/20/2019. The inspector reviewed 4 staff records during the inspection today. Standard #: 22VAC40-185-70-A Description: Based on record review and discussion with the staff, the facility failed to ensure that the staff records contain all of the elements as required by the standards. Evidence: 1. The record for Staff #1 did not contain written information to demonstrate that the individual possesses the education required by the job position. The record for Staff #1 had a documented job title of Program Director. 2. The record for Staff #2 did not contain the following information: verification of age requirement, job title, written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job title, and the health information as required by 22VAC40-185-160. 3. The record for Staff #4 did not contain the following information: verification of age requirement, job title, and the written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job title. Plan of Correction: The Child Care Coordinator will update all files with correct information to meet the standards.
