An unannounced monitoring inspection was conducted to day between 10:50 am and 1:50 pm. There were 80 prechoolers and 74 summer camp students with 22 staff which is within the supervision guidelines. The children were observed watching a musical show, playing with table toys, on the playground and eating lunch. Lunch consisted of a Chicken Patty, three bean salad, peaches and milk. Positive interactions were observed during this inspection. The physical plant including the playground, children's records, staff records, classrooms, fire drill logs, medications, emergency supplies, menu's and medications were inspected. Standard #: 22VAC40-185-130-A Description: Based on a review of children's records, 2 of 5 records have not obtained documentation that each child has received the immunizations required by the State Board of Health before the child first day of attendance. Child #4 and #5 do not have documentation of immunizations by the first day of attendance. Plan of Correction: I will contact and get updated information.
Standard #: 22VAC40-185-140-A Description: Based on a review of children's records, 1 or 5 records show that the center failed to ensure that each enrolled child had documentation of a physical examination by the 30th day of attendance. Evidence: Child #4 does not have documentation of a physical by the 30th day of attendance. Plan of Correction: I will contact the parents and get updated information.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that hazardous substances will be kept in a locked place using a safe locking method. Evidence: 1) In Classroom 6, bleach water, bleach cloths and odor eliminator in unlocked cabinet above changing table. 2) In Room 4, there were bleach wipes and sanitizer on a cabinet. Plan of Correction: I will remind staff to lock all cleaning products away from children.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center failed to ensure that electrical outlets will have protective covers that are of a size that cannot be swallowed by children. Evidence: In Classroom #3 there was an outlet without a protective cover. Plan of Correction: Replaced during inspection.
