Technical Assistance:
Ratios when transitioning children to another classroom were discussed with the program director.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:40 AM and departed at 2:00 PM. Twenty-three children were in care with four staff. Lunch service and nap time were observed. Four staff records and a sample size of five children's records were reviewed. It was reported that no medication has been administered. Injury records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based upon review of four staff records and staff interview, the facility has not ensured that they obtain documentation of a tuberculosis screening for staff at least every two years. Evidence: 1. Documentation in the record of staff 4 indicates a tuberculosis screening dated 1/27/2017. 2. Staff 1 verfied that a more recent screening was not obtained by staff 4.

Plan of Correction: The facility responded with the following: Staff 4 will be required to submit a tuberculosis screening.

Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records and staff interview, the facility has not ensured that the record for each child includes the names, address and telephone numbers for two designated persons other than the parents to be contacted in an emergency when a parent cannot be reached. Evidence: 1. The record provided for child 4, who was in care during today's inspection, did not include the address for one of the two designated contact persons. 2. Staff 1 verified that the address of the designated contact person was not in the record.

Plan of Correction: The facility responded with the following: Child 4 began attending yesterday. The space for the address was highlighted. The parent will be required to provide the address.

Description: Based upon review of written injury records and staff interview, the facility has not ensured that all required documentation in on the written injury records. Evidence: 1. An injury record dated 6/15 2019 for child 6 did not include the time the parent was notified of the child's injury. 2. Staff 1 verified that the time of parent notification was not documented on the 6/15/2019 record.

Plan of Correction: The facility responded with the following: Written injury records will be reviewed to ensure that all required documentation is on the record.
