Comments:
An unannounced monitoring inspection was conducted on 8/19/19 from 11:15 am until 1:05 pm at First Steps Infant Development Center to review the licensing standards. At the time of the inspection 50 children were present with 12 staff present. The sample size consisted of five children's records, five staff's records, three diaper ointments and three injury reports. Children and staff were observed during free indoor play, reading time, lunch, clean-up, nap, parent interaction, transitions and behavioral management. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. In the middle room on the open shelf behind the table was a spray bottle of bleach water.
2. The administrator stated the teacher keeps that out during lunch since they rotate children to eat and clean and disinfect the table between each rotation.

Plan of Correction: The teachers in the middle room will be advised that cleaners must be locked.

Standard #: 22VAC40-185-450-A
Description: Based on observation, the center failed to ensure cots used by children during designated rest period have linens consisting of a top and a bottom cover or a one piece covering which is open on three edges.

Evidence:

1. In the two-year-old room and in the three-year-old room one child in each room did not have a top cover.
2. The administrator stated the child in the two-year-old room does not use a top blanket and the child in the three-year-old room plays with his top blanket so the parents do not send in a blanket for them. She did not realize they were required to have a top blanket.

Plan of Correction: The parents of each child will be asked to bring in a blanket for the children.
