We reviewed requirements for children who are ready to move to a new classroom, including visiting the new class.

The children were playing in centers, writing, reading stories, eating breakfast and playing outdoors. Seven children's files and 11staff files were reviewed. Thank you for your cooperation. Standard #: 22VAC40-185-60-A
Description: Based on the review of seven children's records, the facility failed to collect all required information for each enrolled child. Evidence: 1. There is no emergency contact information on file for child #3. 2. There is no address listed for emergency contacts for child #2, 7. 3. There are no first dates of attendance listed for child #1, 3.

Plan of Correction: The director will ensure that, during the preadmission visit, all lines of the enrollment form are complete. Current files will be reviewed by administrators and all missing information will be requested from parents.

Standard #: 22VAC40-185-240-D-6
Description: Based on the review of 11 staff records, the facility failed to ensure that there is a staff member on duty at all times who holds current certifications on required trainings. Evidence: 1. There is no staff member who holds current daily health observation training.

Plan of Correction: The director is scheduled for an online DHO class on August 21 and will train all staff on August 27 during the staff meeting.

Standard #: 22VAC40-185-270-A
Description: Based on a tour of the outdoor preschool play area, the facility failed to ensure that all areas are safe. Evidence: 1. The cap of one metal fence post is not attached and presents sharp edges.

Plan of Correction: The director will ensure that the maintenance person repairs the fence post. Teachers will be trained to look for broken equipment and materials. Children will not use this area until it is repaired

Standard #: 22VAC40-185-430-C
Description: Based on a tour of the outdoor play area, the facility failed to ensure there are no head entrapment points. Evidence: 1. There is an opening on the preschool climber which allows a 3 1/2 inch by 6 1/4 inch rectangle to fit through but does not allow a nine-inch circle to fit through.

Plan of Correction: The director will ensure that the maintenance person repairs the head entrapment point. Staff will be trained to look for any broken equipment and report it to manager/administrator. The children will not use the climber until it is repaired.

Standard #: 22VAC40-185-500-B
Description: Based on observation of the diapering process, the facility failed to ensure that all diapering regulations are followed. Evidence: 1. In the twos' room staff #1 changed four children's diapers without washing her hands between each diaper change. 2. In the toddler rooms staff #2 changed three children's diapers without washing her hands between each diaper change. 3. Clean baby wipes and hand held computers are being stored on the diapering table in both classrooms. 4. The diapering surface was not sanitized between diaper changes in the toddlers' or the twos' class.

Plan of Correction: The director will train staff on sanitation, health procedures and storage of wipes and computer tablets. The director and assistant director will conduct walk-throughs to ensure that staff are following proper hand washing and diapering procedures.

Standard #: 22VAC40-185-560-M
Description: Based on observation during breakfast, the facility failed to ensure that staff sit with children during meals. 1. Breakfast was observed in three classrooms and there was no staff member seated with children.

Plan of Correction: The director will ensure that staff are trained on interactions with children at mealtimes. Manager/administrator will spot check during meals to ensure compliance. The cook will serve the food and staff will sit with children.
