63.2(17) License & Registration Procedures
An unannounced renewal inspection was completed on 8/16/19. There were 22 children present from five years through nine years with two staff present supervising. The site director arrived for a portion of the inspection. There were no medications at the site. Five children's records and three staff records were reviewed. Four Board officer records and one agent's record was also reviewed for the renewal process. The renewal application has not been received but the agent was aware and had intent to submit it soon. The inspector arrived at the school at 3:10 pm and departed at 5:00 pm. The children were observed during snack, during computer time, and during departures.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on record review and interview, the center failed to ensure that all Board officers who are applicants of the license had a completed fingerprint record as a part of the background check. Evidence: 1. Board officer #4 did not have a completed fingerprint record. 2. Agent #5 verified that this Board officer had not completed the fingerprint process. Plan of Correction: Agent #5 contacted Board officer #4 during the inspection to make an appointment with Fieldprint to submit the fingerprint on Monday.
