Technical Assistance:
Discussed State background check requirements, best practice for playground equipment safety in summer months, staff trainings, children's records and required documentation.

Comments:
An unannounced mandated monitoring inspection was conducted from 9:30 am to 12:30 pm. Children were observed participating in group play and eating snack (english muffins with jelly, fruit and milk). Diapering and hand-washing procedure were observed, and transition from playground to classroom. Ample supplies of materials, age appropriate supplies and equipment. Observation of all three classrooms and playground was conducted. Reviewed 3 staff records, 3 children?s records, medications, evacuation drills, emergency supplies, center policies and physical plant. Thank you for your cooperation. Violations:
Standard #: 22VAC40-185-210-A
Description: Based on record review, staff been used as program leaders files were incomplete. Evidence: Staff #2, who was being used as a Program leader, did not have documentation of program leader qualifications available for review.

Plan of Correction: I will have staff get them for me and will email them to you.

Standard #: 22VAC40-185-240-A
Description: Based on staff record review, staff did not receive child abuse and neglect and the legal requirements for reporting suspected child abuse training by the end of their first day. Evidence: Staff#2 (start date 6/12/19) did not have documentation of child abuse reporting and recognition training.

Plan of Correction: I will have her print it and send it to you.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the playground, areas and equipment of the center, inside and outside, were not being maintained in a clean, safe and operable condition. Evidence: Protruding nails were observed in several places on the playground fence which were accessible to children.

Plan of Correction: I will have them pushed back in to make it safe.

Standard #: 22VAC40-185-430-B
Description: Based on observation, materials and equipment available were not age and stage appropriate as items available could be swallowed or present a choking hazard. Evidence: In the Path Finders (24 month old) room, push pins were being used on all bulletin boards and were accessible to children in the room.

Plan of Correction: These were removed immediately from the room.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, the center did not obtain the results of a search in the central registry for any other state in which the individual has resided in the preceding five years. Evidence: Staff #2 (start date 6/12/19) did not submit within 30 days an out of state central registry check (Maryland).

Plan of Correction: I will have staff send as soon staff is on break.
