An unannounced monitoring inspection was conducted August 13 2019 between 11:00am and 2:00pm. There were 108 children in the direct care of 20 staff. Children were observed being diapered, washing hands, preparing for nap, having tummy time, being read books, and returning from a field trip. The center is clean and organized. The center offers age-appropriate materials in each classroom. Ratios were observed to be maintained throughout the inspection. Standard #: 22VAC40-185-130-B Description: Based on record review, 1 of 7 children's records reviewed did not have documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #7, aged 20 months, last had documentation of an immunization record 12/4/2018Plan of Correction: We will request updated immunization records from parents. Moving forward director team will reach out to parents for updated immunization as needed.
Standard #: 22VAC40-185-150-B Description: Based on record review, 1 of 7 children's records reviewed did not have an immunization record that was signed by a physician, his/her designee, or an official of a local health department.
Evidence: Child #1 record contains a shot record, but it is not signed by the physician.Plan of Correction: We will reach out to parents to reach out to the doctor to sign the record.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence: In Preschool 1 there was a can of shaving cream labeled "Keep out of reach of children" that was on the dining area counter top. In Preschool 2 there was a spray disinfectant that was not locked in a cabinet. In PreK 1, there was a cabinet that was not able to close and lock that contained sunscreen and a cleaning agent.Plan of Correction: Was corrected at the time of inspection. Moving forward we will retrain staff to make sure hazardous substances are locked. We will get a smaller bin to store sunscreen so that the cabinet will close properly.
