Comments:

A Subsidy Health and Safety inspection was conducted on 08/20/2019 from 9:00am to 3:40pm to review supplemental health and safety requirements. The children were observed participating in story time, playing outside and in centers. A formal observation was completed with the infant and twos classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini buses. Eight children?s records and seven employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center?s injury prevention plan was reviewed and was updated 01/2019. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 08/20/2019 Last shelter-in-place drill: 04/13/2019 Last fire inspection: 05/21/2019 Last health inspection: 07/16/2019 Last lock down drill: 09/27/2018 Today, the following child to staff ratios were observed: Infants - 12:3 Toddlers ? 10:2 Twos - 13:1 Preschool and Pre - Kindergarten ? 20:2 Transitional K - 17:2 School Age 11:1 A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. One violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice. Standard #:	22VAC40-665-580-F
Description:	Based on record review, the center did not ensure that two of seven staff files had documentation of a current certification in cpr and first aid. Evidence: 1. The record of staff #3 (start date: 3/26/19) did not have documentation showing that the staff has completed a certification in cpr and first aid. 2. The record of staff #4 (start date: 12/17/18) had documentation showing that the staff had an expired cpr and first aid certification dated 7/6/17-7/6/2019.
Plan of Correction:	We have contacted our first aid and cpr trainer and we will have the staff complete the training by the 13th of September.
