An unannounced Subsidy Health and Safety Inspection (SHSI) was initiated in the center on July 26, 2018, continued on July 31, 2019 and completed on August 19, 2019 regarding a complaint received by the department.
An SHSI supplemental inspection in the areas only related to the complaint inspection was conducted. The results of the inspection were reviewed and verified with the center director during the exit interview. The area of non-compliance is identified on the violation notice. Standard #: 22VAC40-665-550-2 Description: Based on staff interviews and documentation review, the vendor did not ensure to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional
Evidence: Center staff reported child #7 was injured on 7-15-2019 and child #8 was injured on 7-19-19 at the center and both child #7 and child #8 required medical treatment. On 7-26-2019 during inspection the center director confirmed the Department had not been informed of the injury. Reports regarding the injury were submitted to the Department on July 28, 2019.Plan of Correction: The center director will ensure to submit in timely manner in the future.
