Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted August 21, 2019. The on-site inspection was from 12:30p.m. to 4:10p.m. On this date, there were 27 children present in four classrooms and in the care of six staff members. There were two additional staff members available to assist if needed. There were 42 children enrolled in the center. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were listed on the supplemental page. There were five staff members' records and five children's records reviewed. Four classrooms were toured and observations were made in two of those rooms. There were six staff members and four children interviewed. There was no medication on-site. Lunch was brought from home and afternoon snack consisted of chocolate chip cookies and milk. Some of items reviewed and observed during the inspection included: choice time, infant care and feeding, bathroom and hand washing procedures, nap time, afternoon snack, table top activities, dismissal procedures, arrival of school-age children, updated health and fire inspections, required posted information, updated insurance information, documentation of evacuation, shelter-in-place, and lockdown drills, warm water temperature, and first aid kit and emergency supplies. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on the review of five children's records and verification from the director, the center failed to obtain documentation that one child had been adequately immunized before the first day of attendance. Evidence: 1. The first date of attendance for Child #4 was 03/26/2018 and the provider did not obtain documentation of the child's immunizations until 04/19/2018. 2. Staff Member #1 confirmed the record of immunization had not been received before the child's first date of attendance.

Plan of Correction: The provider will ensure that moving forward an immunization record showing a child has been adequately immunized will be obtained before a child attends the center.

Standard #: 22VAC40-185-140-A
Description: Based on the review of five children's records and confirmation from the director, the center failed to obtain documentation of one child's physical examination within one month of the child's first day of attendance. Evidence: 1. The first date of attendance for Child #2 was 02/06/2019 and the provider did not obtain documentation of the child's physical examination until 03/25/2019. 2. Staff Member #1 confirmed the physical examination report was received over a month after the child's first date of attendance.

Plan of Correction: The provider will ensure that moving forward a copy of a child's most recent physical examination will be obtained before a child attends the center.
