A Subsidy Health and Safety inspection was conducted on 8/22/19 to review supplemental health and safety requirements. There were 8 children present with 3 staff. Reviewed 1 staff file, 2 children's files, fire inspection report, emergency evacuation log, attendance, menu, daily activities, and physical plant. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-580-B Description: Based on record review, staff did not have documentation of completing the Pre-Service training within 90 days of hire. Evidence: Staff#1 (hire date 2012) did not have documentation of having completed Pre-Service training. Plan of Correction: I will ensure the training is completed.
