Comments:
A Subsidy Health and Safety inspection was conducted on August 14, 2019 to review supplemental health and safety requirements. Six staff were present with eighty-one children. Eight children staff records and each staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. If you have any questions, contact your licensing inspector at (757) 404-5261. The providers responses for the ?Description of Action To Be Taken? were not received as of August 26, 2019 and will not appear on this ?Violation Notice?.

Description: Based on interview, facility did not ensure there is a current written list of all children's allergies, sensitivities and dietary restrictions that is dated and kept confidential in each room or area where children are present. Evidence: Four groups of children were observed in different rooms/areas. There was not a list of children's allergies, sensitivities and dietary restrictions. Staff reported they do not have a list and it is maintained in the main office.

Description: Based on review, facility did not ensure 1 of 5 staff is certified in first aid training within 90 days of beginning employment. Evidence: On April 30, 2019, the record for staff #4 (start date May 22, 2018) did not have documentation of current first aid certification.
