Comments:
A Subsidy Health and Safety inspections was conducted on 08/20/2019 from 9:50 A.M.- 1:15 P.M. to review supplemental health and safety requirements. At the time of the inspection 69 children were in care with thirteen staff, and two administrators present. Six children records, seven staff records, three medication records and required documentation, evacuation drills, first aid kits, emergency supplies, five injury reports, playground,fire inspection, health inspection,playground safety plan, and posted required information was reviewed. Children were observed during transition to outside play, infant feeding, diapering, interactions with infants, story time, music and movement, behavioral guidance, gross motor play outside, handwashing, lunchtime, and naptime. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Exceptions to compliance are noted on the violation notice.

Description: Based on a review of medications, the center failed to obtain a written allergy plan for a child with a diagnosed food allergy.

Evidence:
1. Child 1M had two emergency medications at the center for a food allergy. There was not a written allergy plan on file.
2. Staff verified that they did not have a copy of the written allergy plan.

Plan of Correction: Administration will contact the parents and obtain a written allergy plan from the physician.
