An unannounced renewal inspection was conducted today from 11:15am-12:30pm. There were 40 children directly supervised by 8 staff. The physical plant, 4 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in circle time, coloring, painting, and infants were observed eating. There was a supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-130-C Description: Based on record review, the provider did not obtain documentation of additional immunizations once between each child's fourth and sixth birthdays. Evidence: Child 4's has an immunization record dated from 2015. The child is 7 years old and does not have an update since Child 4 was three years old. Plan of Correction: I will the parents for updated immunizations.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center did not maintain complete children's records for each enrolled child. Evidence: 1) Child 1 is missing documented date of entry and addresses for emergency contacts. 2) Child 2 is missing one parent work phone number. 3) Child 3 is missing addresses for emergency contacts. 4) Child 4 is missing first date of attendance. Plan of Correction: I will ask parents to complete the forms and get the missing information.
Standard #: 22VAC40-185-520-C Description: Based on observation, the center did not meet all requirements for diaper ointments. Evidence: Child 1's diaper ointment was not labeled with their name, and stored in a backpack accessible to the children in care. Plan of Correction: We will make sure to label it and move to a place that children cannot reach.
