Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, the documentation of a half hour of tummy time, and the monitoring of fraying mats within the Infant 2 room.

Comments:
The following inspection took place between the hours of 9 am through 2:30 pm. Today, I met with the Staff of Primrose School at Moorefield Station located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 133 children with 28 staff; good ratios in all 11 rooms. A total of 10 children?s files, 2 board officers, and 10 staff files were reviewed. The children were engaged in the following: center activities within their rooms, story time, art, games, large motor skills on center playground, small motor skills with cutting paper, lunch, bathroom breaks, hand washing, diapering, nap time, and other organized activities. Standard #: 22VAC40-185-60-A
Description: Based on review, 1 of 10 children's files indicated that not all of the required documentation/information was contained within each child's record. Evidence - 1. On the date of inspection (08/22/2019), the proof of identification for Child #8 was not available for review. 2. The start date for Child #8 was listed as - 07/25/2019.

Plan of Correction: The proof of identification shall be found or obtained and placed on file.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the resilient surfacing located in two separate areas does not comply with minimum safety standards for fall zones under equipment with moving parts. Evidence - 1. On the date of inspection (08/22/2019), the swing set (two swings) located on the preschool playground, and the tire swing located on the school age playground showed areas where the resilient surfacing has worn away; exposing the hard compacted surface underneath. 2. The equipment was not in use at the time of observation.

Plan of Correction: Contractors have reviewed and are scheduled to address these areas. In the meantime, until suitable surfacing has been replaced, the use of this equipment by the children in care shall be restricted.
