Comments:
An unannounced monitoring inspection was completed today. The physical plant, 5 staff files, 7 children's files, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 11:00am-12:40pm. There were 72 children under the supervision of 16 staff. During the inspection, children were observed having tummy time and diapers changed (infants), participating in circle time, singing songs, learning about the letter "A", playing outside on the playground. On the playground, there was a bouncy house that some of the children were observed playing inside. For lunch, the children were served chicken with mixed vegetables, rice, fruit and milk. The interactions between the children and staff were seen to be very engaging, while positive and constructive in nature. There are currently 3 staff certified in MAT (medication administration training). Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Standard #: 22VAC40-185-130-B
Description: Based on file review, 1 out of 7 children's records does not have additional immunizations every six months up to the age of 2 years. Evidence 1. Child #5 is 2 years old. However, the most recent immunization record was updated when the child was 15 months old.

Plan of Correction: The Director emailed the parent during the inspection, requesting this information.

Standard #: 22VAC40-185-240-D-5
Description: Based on interview, the center does not have at least one staff member on duty, who has obtained within the last three years instruction in performing the daily health observation of children. Evidence 1. The Director stated that the staff member who had completed this training left the center a few weeks ago.

Plan of Correction: The Director has a CPR/First Aid class scheduled for September 10th. She will ask the instructor (if they meet the qualifications to teach the training) if she can teach this class to staff.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on file review and interview, the center did not request a search of the central registry maintained by any other state in which an employee has resided in the preceding five years. Evidence 1. Staff #5 has resided in Wisconsin within the past 5 years. No request has been made yet for a central registry search on this employee in that state.

Plan of Correction: The Director will submit that tomorrow.
