63.2(17) License & Registration Procedures

Comments:
An unnanounced monitoring inspection was conducted on August 26, 2019 from approximately 2:30 pm to 5:20 pm. Eleven staff were present with fifty-nine children. Five children and four staff records were reviewed.

Violations:
Standard #: 22VAC40-185-150-B
Description: Based on review, facility did not ensure 2 of 5 children's records has a physical and immunization records that is signed or stamped by a physician, his/her designee or an official of a local health department. Evidence: The records for child #4 (start date August 21, 2017) and child #5 (start date August 12, 2019) had an immunization record that was not signed or stamped by a physician, his/her designee or an official of a local health department. Director confirmed missing information.

Plan of Correction: Director will ask parent for a updated shot record with signature.

Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure 1 of 5 children's records have documentation of required information. Evidence: The record for child #5 (start date August 12, 2019) did not have father's employment phone number, first emergency contact address and a second person to contact in an emergency if a parent cannot be reached. Director confirmed missing information.

Plan of Correction: Director will get parents to fill out electronic forms to ensure all information is documented and current.

Standard #: 22VAC40-185-500-B
Description: Based on observation, facility did not ensure disposable diapers are disposed of in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: Staff member in the 2's classroom was observed changing a child's diaper and disposing of the diaper in a covered trash can. Staff member lifted the lid on the trash can with her hand to dispose of the diaper. Owner confirmed the step was not operable.

Plan of Correction: A new trash can will be purchased for diaper disposal.

Standard #: 22VAC40-191-60-C-2
Description: Based on review, facility did not ensure 1 of 4 staff have a completed central registry check within thirty days of beginning employment. Evidence: The record for staff #4 had documentation of completing orientation training on June 3, 2019 and there is not a completed central registry check. Director confirmed missing information.

Plan of Correction: Director will call central registry unit and ask if the check was received.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review and interview, facility did not ensure 1 of 4 staff have an Office of Background Investigations (OBI) national fingerprinting determination letter prior to beginning employment. Evidence: The record for staff #2 has documentation that orientation training was conducted on August 12, 2019. There is not a OBI fingerprinting determination letter. Staff #2 was observed in the school age room during inspection. Director confirmed missing information.

Plan of Correction: Fingerprint was conducted 8/16/2019. He will not work until the fingerprint letter is received.
