An announced investigation was conducted on 7/11/19 from 9:00am to 10:15am with law enforcement for a self reported incident regarding a forbidden action. As a result of staff interviews and a staff file review, violations were issued in the areas forbidden action, care, protection and guidance and staff qualifications. Standard #: 22VAC40-185-210-A Description: Based on review of Staff A's file and interviews with staff the documentation of education and programmatic experience was incomplete that Staff A was a qualified program leader. Evidence: Staff A with a hire date of 11/2/18 was working as a program leader in the Montessori room with out documentation of education or programmatic experience. Plan of Correction: The staff member provided foreign credentials and was asked to have them translated. The staff member had been employed for 8 months and therefore possessed the programmatic experience. All staff files have been reviewed and are current.
Standard #: 22VAC40-185-340-A Description: Based on interviews and a review of video footage it was determined that staff failed to ensure the care, protection and guidance of children in care. Evidence: on 7/5 19 in the Montessori room at approximately 10:27am Staff A can be seen forcibly grabbing Child A (age 4) by the ankles and throwing the child onto the tile floor. This incident was witnessed by the other children and Staff B. Plan of Correction: The child was laying her back on the floor and kicking the teacher in the stomach. The teacher grabbed her ankles to stop her from kicking her. The manner in which the teacher moved her from the carpeted area to the tile floor is not in accordance with our policies. The incident was self-reported to local authorities and the teacher was terminated. All staff were retrained on positive quidance on 8/12/19.
