Comments:
A Subsidy Health and Safety inspection was conducted on August 27, 2019 to review supplemental health and safety requirements. New subsidy requirements became effective on October 17, 2018. The on-site inspection was from 10:40 a.m. to 2:50 p.m. On this date, there were 66 children present in eight classrooms and in the care of 14 staff members. There were three additional staff members available to assist if needed. There were 126 children enrolled in the center. Nine staff members' records and seven children's records were reviewed. All eight classrooms were toured and observations were made in three of those rooms. Nine staff members and four children were interviewed. The medication and medication paperwork were reviewed for three children.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on review of nine staff records and verification from the director, one staff member failed to obtain a negative tuberculosis (TB) screening at the time of employment.

Evidence:
1. Staff Member #3 obtained a negative TB screening on 07/09/2019. Date of employment for Staff Member #3 was 07/05/2019.
2. Staff Member A confirmed the TB was after the staff member was hired and after coming in contact with children.

Plan of Correction: The director will ensure that all new staff members obtain their TB before being hired and before coming in contact with children.

Standard #: 22VAC40-665-550-2
Description: Based on review of documentation and confirmation from the staff, the center failed to report a serious injury of a child to the department within two business days.

Evidence:
1. The on-site injury to Child AA (age 1 year) occurred on 07/24/2019. The center submitted the serious injury report to the department on 07/29/2019. The child was seen by a physician on 07/24/2019.
2. The staff confirmed the report was not made within the required two business days.

Plan of Correction: Now all staff are aware of the two business day requirement for reporting serious injuries and the center will ensure it will always report within that time frame.
