Comments:
An unannounced monitoring inspection was conducted on Friday, August 2, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 9:45 AM and concluded the inspection at 1:55 PM. The census for today's inspection consisted of 43 children. During the inspection, staff and children were observed dancing, coloring, eating snack, washing hands, playing board games, and napping. The classrooms are decorated with children?s art work and equipped with age and stage appropriate materials. Children's equipment, learning materials, outdoor and indoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Five children's records and five staff records, annual fire and health inspections, fire drill, lockdown and shelter in place drill logs, daily attendance, and all required postings were reviewed.

The director and another member of management were available for the inspection and were present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

The violation notice was amended on September 12, 2019.

Violations:
Standard #: 22VAC40-185-350-E-5
Description: Based on observation, the facility did not ensure compliance with staff to children ratio requirements.

Evidence:
1. The center did not maintain a ratio of one staff to every 18 children for children five years to eight years old.
2. In classroom #4, from approximately 9:57 AM to 10:17 AM there was one staff to 23 children ages five years to 11 years of age.
3. The director stated that the other teacher likes to assist in the kitchen preparing snack for the children.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center did not ensure that the surface of tables were sanitized before and after each use for feeding. Evidence: 1. The tables in classroom #4 were not sanitized after children ate. 2.The children ate snack, threw away trash and began playing board games at the tables.

Description: Based on a review of staff records, the center did not deny continued employment for one staff member when a central registry finding was not obtained within 30 days of employment. Evidence: The record for staff #3 (date of hire 06/10/19) did not contain a completed central registry finding as of 08/02/19.
