Comments:
A complaint investigation was initiated on August 27, 2019 regarding care and protection, supervision, ratios, and false advertising. Interviews were conducted, and documentation and video-camera footage were reviewed. There was not a preponderance of evidence to support the allegation regarding ratios and false advertising; however, the complaint was deemed valid with regard to lack of supervision, and lack of care, protection and guidance. The complaint related violations, and one- non-related violation, are identified in the Violation Notice. Complaint related: No
Description: Based on observation, areas and equipment of the center, inside and outside, are not being maintained in a clean, safe and operable condition. Evidence: A canister light was observed hanging by it's electrical cord in the ceiling of the canopy in the entry way of the center.

Plan of Correction: We will have fixed on Thursday.

Complaint related: Yes
Description: Based on interviews with staff and record review, staff who were supervising children did not ensure their care, protection, and guidance. Evidence: 1. On 8/14/19, from 4:52 pm to 5:05 pm - Child#1, (4 yr old) was observed on video footage, being allowed to remain in a bathroom for 13 minutes unsupervised and without assistance for a bathroom medical condition . Parent provided documentation regarding a bathroom medical condition and accommodations on 8/7/19 which were to be shared with all child's caregivers prior to child's first day of care (8/12/19) . Interview with Staff#1 and Staff#2 revealed they were unaware of Child#1's bathroom medical condition or accommodations at the time of the incident. 2. Staff#1 was unaware of Child#1's name or location when Child#1's parent came to pick up. Parent located Child#1 in the bathroom unsupervised and without assistance for a bathroom medical condition.

Plan of Correction: Upon this coming to the attention of the leadership team, a new policy was implement immediately. The bathroom door was made into a half door for easier supervision. The teachers were re-trained on transitions. The child returned to school returned the next day.

Complaint related: Yes
Description: Based on staff interviews and observation of the video footage, staff did not check on children under 10 years of age who has not returned from the rest room after five minutes. Evidence: Child#1 was observed on video going into the bathroom in the Hong-Kong room at 4:52pm. Staff did not check on child until parent arrived at 5:05 for pick up.

Plan of Correction: Standard operating procedure will be that staff check on children in the bathroom after 3 minutes. Training will happen today.
