Standard 340.F discussed at the request of the program director- Staff under the age of 18 can not be left alone with children during nap. Reminder: Two shelter practice drills are required annually
An unannounced monitoring inspection was conducted on 08/26/2019 from 10:30am - 1:50pm. At the time of entrance there were 11 children in care with three staff. The children ranged in age from infant - four years. Children were observed during outdoor play, classroom activity time, lunch and nap. Records reviewed for four staff and six children. No medication is being administered at this time. Violations were observed in five parts of the standards and are listed on the violation notice. These violations were reviewed with the program director at the conclusion of the inspection.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that children's records contain all of the required elements. Evidence: 1. The record for child 1 lacked the following required information: a. Work phone number for each parent's place of employment; b. Street address for two emergency contacts; c. Documentation of viewing proof of the child's identity and age. 2. The record for child 2 did not have documentation of annual review and updated by the parent. a. The record was last updated by the parent of child 2 on 01/01/2018. 3. The record for child 3 did not have documentation of annual review and updated by the parent. a. The record was last updated by the parent of child 3 on 03/01/2018. Plan of Correction: All files have been updated.
Standard #: 22VAC40-185-260-A Description: Based on record review, the center failed to provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: 1. The most recent fire inspection for the facility was conducted 07/02/2018 according to documenation on file at the center. 2. The program director confirmed this as the most recent fire inspection from the City of Chesapeake Fire Marshall's Office. Plan of Correction: Called fire inspector for new inspection.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe condition. Evidence: 1. The wooden bridge, located on the playground, has a missing board in the middle of the walkway. This is a trip hazard. 2. One storage shelf, in the preschool classroom, has not been secured to the wall. This is a toppling hazard. 3. A cord was observed dangling down from an echo speaker in the infant/toddler classroom. The cord was dangling down to the floor and was within reach of children. This is an entanglement hazard. 4. A metal storage unit in the hallway was not secured to the wall. Children must pass through this hallway when entering and exiting the infant/toddler classroom. This is a toppling hazard. Plan of Correction: -All furniture will be secured immediately. -Wooden bridge was fixed. -Echo speaker removed.
Standard #: 22VAC40-185-440-A Description: Based on observation and interview, the center failed to ensure that cribs shall be provided for children during the designated rest period. Evidence: 1. There was no crib available for one infant ( child 1) in care. 2. Staff 1 stated that it was the practice of the center to hold child 1 during nap time. a. Staff 1 and staff 2 were observed holding child 1 during nap time. 3. Staff 3 confirmed that a crib had not been set up in the infant room for child 1. Plan of Correction: Crib will be set up for use.
Standard #: 22VAC40-185-500-A Description: Based on observation, the center failed to ensure that child and staff hands are washed with soap and running water after toileting and diapering. Evidence: 1. Staff 1 failed to wash her hands after completion of a diaper change for child 1 at approximately 11:00am. 2. Staff 1 failed to wash the hands of child 1 after completion of his diaper change at approximately 11:00am. Plan of Correction: Diaper policies will be reviewed with staff.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center failed to ensure that the diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. *Sanitized means the surface of the item shall be sprayed with the disinfectant solution and allowed to air dry. Evidence: 1. Staff 1 failed to clean the diapering table with soap and water after completion of a diaper change observed at approximately 11:00am. 2. After completion of this diaper change staff 1 sprayed the diapering surface with disinfectant solution however, the surface was not allowed to air dry. a. Staff 1 immediately wiped the surface with a paper towel after the disinfectant was sprayed on the table. Plan of Correction: Diapering policies will be reviewed with staff.
Standard #: 22VAC40-185-550-M Description: Based on record review, the center failed to ensure that the written record of children's serious and minor injuries shall include all of the required elements. Evidence: Two of two injury reports reviewed lacked documentation of any future action to prevent recurrence of the injury. Plan of Correction: Will discuss with staff how to completely fill our reports.
Standard #: 22VAC40-185-560-G Description: Based on observation, the center failed to ensure that when food is brought from home the food container shall be clearly dated and labeled in a way that identifies the owner. Evidence: One Tummee Tippe bottle of milk was not dated and labeled in a way to identity the owner. This bottle was observed in the infant/toddler classroom. Plan of Correction: Will discuss with parents our policies for labeling all cups and to not let child walk in with them.
Standard #: 22VAC40-185-570-B Description: Based on observation, the center failed to ensure that bottles shall not be used while the child is in his designated sleeping location. Evidence: Child 3 was observed resting on her rest mat with her tummee tippe bottle of milk. Plan of Correction: Will discuss with parents to see if child can sleep with something else.
Standard #: 22VAC40-185-570-E Description: Based on observation, the center failed to ensure that prepared infant formula shall be dated and labeled with the child's name. Evidence: One bottle of prepared infant formula, observed in the infant/toddler classroom, was not dated or labeled with the child's name. Plan of Correction: Will discuss with parents our policies for leaving bottles laying around.
