Discussed with Administration: Children transitioning into classrooms.
An unannounced monitoring inspection was conducted today. Children were observed transitioning from the playground, washing hands, eating lunch, during diaper changes, and resting quietly. 3 staff and 4 children's records were reviewed. See Violation Notice form missing information. Emergency supplies, emergency evacuation drills, shelter-in-place drills, first aid supplies, infant bottles labeled, menu and cot/rest mat spacing are all in compliance. Standard #: 22VAC40-185-130-B Description: Based on a review of records, immunization records have not been updated every six months for children under the age of 2 years. Evidence: The immunization records for Child #1 (Date of Birth (DOB), 12/27/17) dated 1/2/19 is not signed, stamped or electronically signed by the physician. There is no update at approximately 6 months later for the 18 month check up. (IPV #4 is due) Plan of Correction: I will have the parents bring it in by Friday.
Standard #: 22VAC40-185-60-A Description: Based on a review of records, one child's record does not contain all required information. Evidence: Missing information: documentation of a signature of the staff person viewing the proof of birth document. Plan of Correction: I will make sure that is completed.
