Technical Assistance:
Discussed age limits on new playground equipment, choking and inhale-able items for children under 3 yrs, and best practices for diapering.

Comments:
A mandated unannounced Monitoring inspection was conducted today from 10:00 am to 2:45 pm. There were 154 children present with 30 staff. Children were observed painting, coloring, putting puzzles together, cleaning up, outside on the new playground, being held while having a bottle, having tummy time, diapering and toileting, and transitioning. Reviewed 10 children's files, fire and health inspections, emergency evacuation log, menu,outdoor play areas and physical plant. Violations of the Standards are recorded on the Violation Notice. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on observation, the licensee did not ensure that the center's activities, services, and facilities are maintained in compliance with the terms of the current license issued by the department.
Evidence:
Room 105, which is designated for children 2 1/2 years and above was being utilized by children ages 2 1/2 years and younger.

Plan of Correction: We will be closing that room until further notice

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete.
1. Child#4, Child#6, Child#7 and Child#10 were missing the names of persons authorized to pick up the child.
2. Child#7 was missing the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: We have contacted the parents for the information

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center, inside and outside,are not being maintained in a clean, safe and operable condition.
Evidence:
1. In Rooms 202 and 205, there are missing ceiling tiles around the HVAC vent in the middle of the rooms.
2. In Room 204, the emergency exit door is sticking and hard to open.
3. The faucet in the downstairs girls bathroom is missing the handle.

Plan of Correction: We will speak with maintenance to have them address the issues.

Standard #: 22VAC40-185-330-B
Description: Based on observation, resilient surfacing on the playground does not comply with minimum safety standards.
Evidence:
The Infant and Toddler playground does not have the minimum depth of 6 inches of resilient material under the equipment exits and fall zones.

Plan of Correction: We will purchase more mulch

Standard #: 22VAC40-185-500-B
Description: Based on observation, the diapering surface is not being used only for diapering or cleaning children, and it is not being cleaned with soap and at least room temperature water and sanitized after each use.
Evidence:
1. In Room 214, the children's diapering schedule, results and pen were being stored on the diaper changing table.

Plan of Correction: We re-train staff on not storing things on the table

Standard #: 22VAC40-185-560-G
Description: Based on observation, food brought from home was not clearly dated.
Evidence:
In room 104, children's food items brought from home did not have dates on them.

Plan of Correction: We will re-train staff on this process and remind parents to comply.
