Technical Assistance:
We discussed the requirements for infant formula record, food safety and hand washing before meals. The risk assessment for violations were provided.

Comments:
The unannounced inspection conducted from 11:25 AM to 1:45 PM was for the purpose of investigating a complaint regarding infant feeding in sleep spaces, use of food withdrawal during behavior management and the care, protection and guidance of children. After observation of the daily routines, interviews with seven staff, and pertinent record reviews, it was determined that the complaint was not valid. Although, there were not any complaint related violations, findings regarding the infant formula records, food safety for children under age three years, and hand washing before lunch were found in violation of licensing standards. The violation report includes the plans for correction. As a reminder, the maintenance of compliance with minimum standards is the responsibility of the licensee.

Description: Based on observation of the lunch routine, and an interview with staff, the center staff did not ensure that all children's hands were washed with soap and running water or disposable wipes before eating meals. Evidence: During the end of the lunch routine, a three year old child arrived for care and was instructed by staff member 7 to sit at the table for lunch. Staff member 7 placed a lunch plate in front of the child who began eating. The child was not sent to wash hands until the licensing inspector questioned the routine for hand washing before children eat.

Plan of Correction: The administrative staff will review with the staff the requirement for making sure that children who arrive after a meal time has started are sent to the sink to wash hands before they are seated at a table and served food. Hand washing will be required before children eats lunch and snacks.

Complaint related: No
Description: Based on observation of the lunch food served to children, and an interview with staff, the center failed to ensure that children under three years in age were not served foods that are known choking hazards. Evidence: The children in the older infant and toddler/two year old areas were served fruit cocktail that included whole grapes. When asked about the grapes, the teachers confirmed that the grapes were not cut into smaller pieces for the younger children.

Plan of Correction: Administrative staff will make sure all staff understand that whole grapes will need to be cut before they are served to children, including in fruit cocktail. The administrative staff will review foods that have been identified as known choking hazards with the staff who work with the youngest ages.

Description: Based on a review of the records for children on formula, and interviews with staff, the center failed to maintain a record of each child on formula, including the brand of formula, feeding schedule and amounts. Evidence: The licensing inspector asked for the formula feeding schedule record for child 1F. A record was not available in the infant room or the child's file. There was only a canister of commercial formula with "6 oz every 3 hours" hand written on the lid.

Plan of Correction: Administrative staff will develop a place for parents to add information about feeding commercial formula, the amounts and frequency to be added to the contract record and changed as needed. The center will maintain records of children fed formula.
