Technical Assistance:
22 VAC 40-185-270 (building maintenance) and 22 VAC 40-185-440 (rest mats) was discussed with center administration during the inspection.

Comments:
An unannounced mandated inspection was conducted on 8/29/19 from 11:00am until 12:30pm. At the time of the tour, there were 55 children in care with 10 staff and 2 administrators present. A sample of 5 children's records and 8 staff records were reviewed. Children were observed participating in morning activities and eating lunch. Diapering, restroom, hand washing and lunch service procedures were also observed. Medication, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the areas of administration and physical plant are identified on the violation notice and were discussed with the center owner during the exit interview.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center did not ensure that staff records contain all the required information.

Evidence:
1. The record for staff 1 contains documentation of only one reference as to character and reputation as well as competency, where two are required.
2. The record for staff 2 contains no documentation that two or more references as to character and reputation as well as competency were checked before employment.
3. The record for staff 3 does not contain written information to demonstrate that the individual possesses the orientation training required by the job position.
4. The center owner confirmed that the records for staff 1 and 2 are lacking reference checks and the record for staff 3 is lacking documentation of orientation training.

Plan of Correction: Reference checks will be completed for staff 1 and 2 by 9/4/19. Staff 3 completed orientation training; however, center administration failed to document the training in staff 3's record. The orientation training form will be filled out and signed today and placed in staff 3's record. Going forward, administration will ensure that staff records contain all the required elements.

Standard #: 22VAC40-185-290-2
Description: Based on observation, the center did not ensure that when serving children of preschool age or younger that fans shall be out of reach of children.

Evidence:
There was a box fan being used in the toddler room that was within reach of children in care. The fan was sitting on a low shelf by the door and children were observed playing beside the fan.

Plan of Correction: The fan was removed during the inspection. Going forward, the center will ensure that if fans are used they are placed out of reach of the children.
