A Subsidy Health and Safety inspection was conducted on (insert date) to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found. Please contact me if you have any questions at Stacy.Doyle@dss.virginia.gov or 571-835-0386.
