Comments:
An unannounced monitoring inspection was conducted between the hours of 8:00am and 10:00am. Upon arrival there were 72 children in care with 17 staff present. A sample of 7 children's and 4 staff records were reviewed in addition to 4 medications. Infants were observed sleeping in cribs and playing on the floor with staff. Preschool children were observed during circle and on the playground.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure areas and equipment of the center, were maintained in a safe condition.
Evidence: There was peeling paint along the entire window ledge in the school age room and a patch of peeling paint in the twos room that measured about 2 inches in diameter.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
A work order will be placed today and will be followed-up with by the director.

Standard #: 22VAC40-185-440-H
Description: Based on observation the center did not ensure all crib requirements were met.
Evidence:
An infant (child #8) was observed sleeping in a crib. The mattress that infant was laying on curled-up on one end creating a 2 inch space between the mattress and the crib.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director will remind staff to flatten the mattresses before babies are placed in the cribs. The director will also order new crib sheets that have a better fit.

Standard #: 22VAC40-185-500-B
Description: Based on observation the center did not ensure all requirements were met when diapering.
Evidence:
A staff in the twos room was observed using the diapering surface to sort and label new diapers and supplies that had just come into the classroom.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director will remind staff that no other activities, with the exception if diapering or cleaning children shall be done on a diapering surface.
